Special Education Removal of Limitations and Approval Applications

Special Education Areas
LBS1 Limitation Removal

An applicant holding a special education LBS1 Limited certificate endorsed in one or more of the following areas: Learning Disabilities, Social Emotional Disorders, Educable Mentally Handicapped, Trainable Mentally Handicapped of Physically Handicapped and seeking an evaluation of transcripts for removal of limitations will complete the ISBE Form 73-85 (pdf). There is no fee required for this request. The application should be submitted along with any applicable official transcripts, in unopened, sealed envelopes, or other supporting documents to:

*To be official, transcripts must be submitted in the sealed envelope from the college or university or be sent directly by the institution. Transcripts received that are not in a sealed envelope from the university or sent directly from the university will be considered unofficial and cannot be used for evaluation purposes.

Early Childhood Special Education Approval Areas

An applicant holding an Early Childhood certificate and seeking an Early Childhood Special Education Approval should complete ISBE Form 73-26 (pdf). There is no fee required for this request. The application should be submitted along with any applicable official transcripts, in unopened, sealed envelopes, and/or other supporting documents to:

Bilingual/ESL Special Education Approval

An applicant holding an Early Childhood, Elementary, Secondary or Special certificate and seeking a Bilingual/ESL Special Education Approval should complete ISBE Form 73-67 (pdf). There is no fee required for this request. The application should be submitted along with any applicable official, in unopened, sealed envelopes, or other supporting documents to:
