Forms Required When Requesting Building and/or Occupancy Permits and Statement of Completion
For Building Permits, the following are required:

o Form 36-10, Application for Building Permit
o Form 36-11, Plan Review Statements (If not applicable, indicate NA)
o Form 36-35, Confirmation of Plan Review Records (If not applicable, indicate NA)
- One paper copy of the…
o Plans/Specifications – half size
o Project Manual
- One CD-ROM that contains the plans/specifications and project manual
Please do not place paper labels on the CD as data will be lost if the label comes off. Information regarding the contents of the CD may be placed on a separate label which can be attached to the case/envelope.
For Occupancy Permits, the following are required:

o Form 36-36, Inspection Statements (If not applicable, indicate NA)
o Form 36-37 Confirmation of Called Inspection Statements (If not applicable, indicate NA)
For Temporary or Mobile Unit Facility Occupancy Permits, the following are required annually:

Please be sure all required signatures/seals are included on these forms.
It is the responsibility of the local School Board to ensure that no facility is occupied before the Regional Superintendent has issued a Certificate of Occupancy.
